Absolute Failure: When Engineering, Enforcement, and Education Don't Work

What can educators and fire prevention personnel learn from failure? A fire fatality represents a failure of one or more of the foundational principles of fire prevention. This session will examine fire fatalities to learn how these failures assist in improving life safety messages and evaluating fire prevention efforts. The presentation will focus on post-fatal fire investigations, examining human behavior, fire protection features, educational messaging, and application, and enforcement of life safety codes. Participants will be engaged in a discussion of real-world fire scenarios and the decisions and factors that resulted in the loss of life.

Lynn Schofield is the Fire Marshal in Provo, Utah. He currently serves as the Chair of the NFPA Education Section. He has presented several times at the NFPA Conference and Exposition, including "Public Education 101, The 3 E's: What Your Mother Didn't Tell You," "So Sue Me, So Sue Me II: the Court of Appeals," "The Origin and Cause of the Provo Tabernacle Fire," and "Investigating the Provo Tabernacle Fire and Boulders Apartment Fire using NFPA 921." Mr. Schofield holds aa MS in Public Administration from Brigham Young University and is a graduate of the National Fire Academy Executive Fire Officer Program. He is an IAAI-certified Fire Investigator, an NAFI-certified Fire and Explosion Investigator, and has qualified as an expert witness. Mr. Schofield is the Past President of the Fire Marshals Association of Utah, and the 2nd Vice President of the Utah Chapter of the International Association of Arson Investigators. He has served at Provo Fire and Rescue for 24 years.
