This case study will detail the findings from the federal investigation of the April 2013 West Fertilizer Company fire and explosion that occurred in West, Texas, killing 12 emergency responders. The presenter - an investigator with the U.S. Chemical Safety Board will explain 4 key factors that contributed to the fatalities, share the challenges the emergency responders encountered, and 'lessons NOT learned' from previous incidents. The case study will also address how the use of NFPA 471, Recommended Practice for Responding to Hazardous Materials Incidents, and NFPA 1620, Standard for Pre-Incident Planning, could help emergency responders to be well-prepared in dealing with fires and accidents at industrial facilities.

Samuel Oyewole, U.S. Chemical Safety Board Dr. Samuel Oyewole, PhD, is a Chemical Incident Investigator at the US Chemical Safety and Hazard Investigation Board in DC. Previously, he was a Professor of Environmental Health and Safety Engineering in the Department of Energy and Mineral Engineering at Pennsylvania State University, where he earned his PhD in Industrial and Manufacturing Engineering. He is a member of ASSE, the Society for Industrial and Applied Mathematics, and the Human Factors Engineering Society. Dr. Oyewole is the author of many technical papers, publications, and speeches.
